2019 SAPSASA Consent Forms

This year we are converting all of our SAPSASA consent forms to online/digital. This means that all SAPSASA event consent forms will now be on QKR! only and will need to be filled out through the app instead of on paper. This will be a more efficient way for us to collect consent forms and easily keep track of who has returned their forms. Notes will still be sent home in paper form informing you of the event details but the consent form will be online only. We understand that some parents do not have the QKR! app or are not familiar with it. Please see the Front Office staff for any assistance with this.
